COVID Vaccine ages 5-11
The COVID Vaccine is distributed in 10 dose vials which must be used within a couple hours of opening the vial. This is the reason we cannot make the shot available at individual well visits or upon demand.
We have already held 3 large clinics and have vaccinated about half of our patient population in this age range in a one week time frame. We only have a couple clinics with availability remaining (please see the dates below).

Who:
- Brighton Pediatric patients who will be age 5-11 on the date of vaccination.
What:
- The Pfizer COVID vaccine 10-micro-gram dose designated for children in this age group.
Where:
- Here at the Brighton Pediatrics office - only during the times listed below
When:
- Friday, November 19th from 5:30 - 7:15 p.m. (next dose Friday, December 10th)
- Tuesday, December 7th from 3:00 - 4:15 p.m. (next dose Tuesday, December 28th)
Please note, the second dose will automatically be scheduled for you based on the date and time you select for the 1st Dose clinic.
How to Schedule:
- Scheduling must be done via our patient portal.
- Click on the Patient Portal Tab here on our website. We recommend using a tablet, laptop or computer. You may use a mobile device, however, you must navigate to the portal via a web browser instead of using the mobile app. The mobile app does not allow you to view available appointments and will give you a false message that "no appointments are available".
- After signing in, you may access scheduling by clicking on the "Schedule Now" button or by clicking on "Appointments".
- Under the "I'd like to schedule a new appointment", select "COVID-19 Vaccine" and click "Find Appointments".
- Each child receiving the vaccine needs an appointment.
Clinic Process:
- We recommend completing this form prior to arriving for your appointment. Please bring a print out of the completed and signed form with you to the appointment. Do not submit the form online as it will go to the state of New York instead of our office. A completed and signed form is required for each child receiving the vaccine.

- Enter our building on the day of the clinic; there is no need to call from the parking lot.
- If you have not completed the forms ahead of time, please take a blank form(s) from the table in our waiting area to complete. Please complete the form(s) in your vehicle. You may re-enter the building as soon as the form is complete.
- Please only approach the check-in window after your form(s) are completed. These forms serve as your "check-in" for the appointment.
- If there are multiple siblings receiving the vaccine, we will take you back to the room in a family grouping.
- Lynn Wilson, RN and Dr. Wirt will be giving the vaccines so the children will be seeing familiar faces.
- After the vaccine is administered, you will be escorted back to the waiting area for the required 15 minute observation period. Patients who have allergies which require them to carry an epi pen, will have to be observed for a total of 30 minutes.
- When the time elapses for observation period, you will be given the CDC card with the first dose marked on it. Please bring the card back to our office for the second dose.
